  • Violation of Code: [.04(4)(c)1(i)-(iii),(v),(viii) ] Raw shelled eggs and raw scallops stored above sauces in prep cooler. Raw chicken stored on top of raw beef in walk-in freezer. Raw shrimp stored on top of cooked pork in walk-in freezer. Raw shrimp stored above vegetables in walk-in cooler. Foods reorganized. Raw animal foods and ready-to-eat foods shall be properly seperated and stored to prevent cross-contamination.  Corrected On-Site.  New Violation.
  • Violation of Code: [.04(6)(f) ] Cold-held potentially hazardous foods not maintained below 41oF; no time controls/documentation in place. Foods discarded. See temperatures marked '*'. Cold-held potetially hazardous food shall maintain a temperature of 41F or below.  Corrected On-Site.  Repeat Violation.
  • Violation of Code: [.07(6)(b) ] Containers of pink lotion detergent not labeled with common name. Container labeled. Containers of cleaners and chemicals shall be properly labeled with common name.  Corrected On-Site.  Repeat Violation.
  • Violation of Code: [.04(4)(d) ] Small containers of sugar, salt and other seasonings not labeled with common name. Containers labeled. Food contaiers shall be properly labeled with common name.  Corrected On-Site.  New Violation.

  • Violation of Code: [.03(2)(a)-(n) ] Failure of Certified Food Manager (CFSM) to perform duties of ensuring Food Code regulations are being followed. Seven (7) critical violations observed during Health Inspection. The responsibility of the person-in-charge or CFSM shall ensure Food Code regulations are being maintained.  New Violation.
  • Violation of Code: [.03(5)(b) ] Employee, while changing gloves, rinsed hands under water using no soap. Later employee properly washed hands, but then dried hands using apron rather than disposable towel. Discussed with employee proper handwashing procedures and had hands rewashed. Hands shall be washed with warm water and soap for 20 seconds and dried with disposable paper towel, continuous towel system or heated drying system.  Corrected On-Site.  New Violation.
  • Violation of Code: [.03(5)(j)1&2 ] Employee drink (canned energy drink) stored above food in Delfield Cooler above food items. Drink discarded. Employee drinks shall be in disposable cup with lid and straw and properly stored to prevent contamination of food, food prep and food equipment.  Corrected On-Site.  New Violation.
  • Violation of Code: [.06(2)(o) ] Multiple plastic lids for food containers stored in handsink in rear of facility. Package of biscuits stored in handsink in rear as well. Lids and package of biscuits removed. Handsinks shall be kept accessible at all times and used for Handwashing purposes only.  Corrected On-Site.  New Violation.
  • Violation of Code: [.04(4)(c)1(i)-(iii),(v),(viii) ] Observed employee handle raw bacon and raw beef at same time, resulting in raw bacon being handled with same gloves as the raw beef had been handled with. Raw bacon cooked to minimum 155F. Discussed with General Manager proper handling practices. Raw animal foods shall be properly handled and seperated during preparation.  Corrected On-Site.  New Violation.
  • Violation of Code: [.05(7)(a)1 ] Food utensils and skillets observed with food residue and grease buildup in storage drawers under grill. Equipment cleaned before being stored. All equipment rewashed. Food-contact surfaces shall be cleaned to sight and touch.  Corrected On-Site.  New Violation.
  • Violation of Code: [.04(6)(f) ] Cold-held potentially hazardous foods not maintained below 41oF; no time controls/documentation in place. See temperatures marked '*'. Foods placed in walk-in to cool (last temped 3.5 hours). Foods below 41 by 11am. Cold-held potentially hazardous food shall maintain a temperature of 41F or below.  Corrected On-Site.  Repeat Violation.
  • Followup inspection will occur within 10 days. Required Additional inspection will occur within 30-60 days.

Boxes of cheesecake stored on top of boxes of raw shrimp in lying down freezer. Raw shrimp stored above cooked foods in 2-door cooler and reachin freezer. Foods reorganized. Raw animal foods and ready-to-eat foods shall be properly seperated and stored to prevent risk of cross contamination.  Corrected On-Site.  New Violation.
  • Violation of Code: [.04(4)(c)1(iv) ] Raw chicken, raw pooled eggs and cooked chicken not stored covered in walk-in cooler. Foods covered. Foods shall be stored covered.  New Violation.
  • Violation of Code: [.04(6)(g) ] Cooked chicken, egg rolls, cooked pork and crab rangoons not datemarked after being cooked and/or removed from original commercial packaging. Refrigerated, ready-to-eat, potentially hazardous food prepared and held in the establishment for more than 24 hours shall be clearly marked to indicate the date by which the food must be consumed, sold, or discarded and held no longer than 7 days. (ex. 2/21-2/27) Foods that are frozen must be marked with date placed in freezer.  New Violation.
  • Violation of Code: [.04(4)(q) ] Container of soy sauce stored on floor in kitchen. Container placed on crate. Food shall be stored at least 6" off the floor.  Corrected On-Site.  New Violation.
  • Violation of Code: [.04(4)(g) ] Fruit/Vegetable sink had remnants of raw chicken and raw beef in sink at beginning of inspection. Sink cleaned and sanitized. Fruit/Vegetable sink shall be used to WASH FRUITS/VEGETABLES ONLY.  Corrected On-Site.  Repeat Violation.
  • Violation of Code: [.04(4)(k) ] Bowls with no handles stored in sauces and spices (ie. flour and sugar) between uses. Scoops stored with handles in contact with food between uses. Bowls removed and scoops placed with handle above food. While not in use, food preparation/dispensing utensils shall be stored with their handles above the top of the food.  Corrected On-Site.  Repeat Violation.
  • Violation of Code: [.05(7)(d) ] Nonfood-contact surfaces of equipment not cleaned at a frequency necessary to preclude accumulation of soil residues. Sides of wok station and fryers contain heavy grease and food residue buildup. Nonfood contact surfaces of equipment shall be cleaned at a frequency to prevent soil/food debris buildup.  Repeat Violation.
  • Violation of Code: [.07(5)(a),(b) ] Floors under equipment contain grease and food debris buildup. Walls, floors and ceilings shall be maintained in good repair and cleaned frequently to prevent buildup of soil, grease and food debris.  New Violation.
  • Violation of Code: [.07(2)(m) ] Rear screen door contains large air gaps that would allow vermin and pests to enter facility. Keep rear door closed or fix air gaps on screen door.  Repeat Violation.

Tomatoes placed in walk-in freezer. Ensure proper cooling methods are used for differing potentially hazardous items for proper cooling time/temperature.  Corrected On-Site.  New Violation.
  • Violation of Code: [.04(4)(q) ] Two (2) large containers of ice cream stored on floor in walk-in cooler. Food shall be stored at least 6" off the floor.  New Violation.
  • Violation of Code: [.04(4)(n) ] Single-use gloves were used to handle raw hamburger patties, then employee proceeded to handle food containers and food utensils. Gloves changed. Single-use gloves shall be changed when performing new tasks.  Corrected On-Site.  New Violation.
  • Violation of Code: [.05(2)(c),(d) ] Fryer baskets contain loose wires. Food contact surfaces shall remain in good repair.  New Violation.
