  • Violation of Code: [.03(4)(a)-(f) ] Person in charge not aware of Employee Health Policy (5 symptoms/5 illnesses) and actions that shall be taken by person in charge. Person in charge made aware of employee health policy. Person in Charge and employees shall be made aware of symptoms and illnesses to ensure proper exclusion/restriction takes place.  Corrected On-Site.  New Violation.
  • No soap at handsink in kitchen. Handsoap and papertowels shall be supplied at all times at all handsinks to ensure proper handwashing.  New Violation.
  • Violation of Code: [.04(6)(f) ] Cold-held potentially hazardous foods not maintained below 41oF; no time controls/documentation in place. See temperatures marked '*'. Food discarded. Cold-held potentially hazardous food shall maintain a temperature of 41F or below.  Corrected On-Site.  New Violation.
  • Violation of Code: [.04(6)(i) ] Time controls not documented according to written procedure. No stickers on pans of food held on time control as according to written procedure. Sticker placed on pans. Time control shall be documented according to written procedure.  Corrected On-Site.  Repeat Violation.
