Mold Found in Ice Machines at Three Restaurants and Other Violations Noted During Recent Health Inspections

A look at the most recent restaurant inspections by the East Metro Health District.

Observations and corrective actions:

  • Violation of Code: [.03(5)(j)1&2 ] Employee drink, without a straw, stored on the prep table, next to empanadas. Employee drink, without a lid & straw, stored on top of clean plates. Drinks were discarded & plates were taken to dish sink at the time of inspection. Employees shall consume food only in approved designated areas separate from food preparation and serving areas, equipment or utensil areas, clean linens, and food storage areas. Drinks shall be in a single service beverage cup with a secure lid and straw  Corrected On-Site.  New Violation.
  • Violation of Code: [.04(4)(m) ] Wiping cloths stored in 0ppm chlorine sanitizer in the sanitizer bucket. Sanitizer was changed to approved concentration at the time of inspection. Ensure sanitizer solutions are at approved concentrations.  Corrected On-Site.  New Violation.
  • Violation of Code: [.10(2)(g) ] Most current inspection report not posted at the time of inspection. The most current inspection report shall be prominently displayed in public view at all times, within fifteen feet (15') of the front or primary public door and between five feet (5') and seven feet (7') from the floor and in an area where it can be read at a distance of one foot (1') away.  New Violation.
  • Remarks: Raw shell eggs stored above cut vegetables in the upright cooler. Eggs moved below vegetables at the time of inspection. Store raw animal foods below/away from produce/cooked/ready-to-eat foods. Ambient air temperature in white cooler was 48F. Cooler turned down & ambient air temperature measured 43F. Ensure ambient air temperature is 41F in all coolers.

  • Violation of Code: [.05(7)(b) ] Ice machine contains mold-like substance buildup around chutes. Machine cleaned every three (3) months according to Certified Food Manager (CFSM). Increase cleaning frequency to avoid soil buildup on food-contact surfaces.  Corrected On-Site.  New Violation.
  • Violation of Code: [.04(6)(f) ] Cold-held potentially hazardous foods not maintained below 41oF; no time controls/documentation in place. See temperatures marked '*'. Food discarded. Cold-held potentially hazardous foods must maintain a temperature of 41F or below.  Corrected On-Site.  Repeat Violation.
  • Violation of Code: [.04(6)(f) ] Hot-held potentially hazardous foods not maintained above 135oF; no time controls/documentation in place. See temperatures marked '**'. Food reheated. Hot-held potentially hazardous food must maintain a temperature of 135F or higher.  Corrected On-Site.  Repeat Violation.
  • Violation of Code: [.04(6)(d) ] Potentially hazardous foods prepared from products at ambient temperature (i.e. canned products) not cooled to 41oF within 4 hours. See temperatures marked '***'. Food discarded. Potentially hazardous foods must be cooled wihtin time/temperature guidelines. Cooled from 135F to 70F within two (2) hours and 135F within a total of six (6) hours.  Corrected On-Site.  New Violation.
  • Violation of Code: [.04(6)(e) ] Pico and diced tomato being cooled using cold-hold line coolers. Temped at beginning of inspection at 48F. Approximately an hour later Pico and diced tomato were still at 48F. Black beans being cooled in walk-in cooler in deep containers and pllaced on cooling rack with no seperation between pans to allow for adequate air circulation. Beans placed in freezer. Discussed with CFSM adequate cooling methods for pico, diced tomato, black beans and salsa. Ensure adequate cooling methods are used when cooling potentially hazardous foods.  Corrected On-Site.  New Violation.
  • Notes--In 'On The Border' Culinary Manual, the CCP (Critical Control Point) noted for Salsa is 41F. Violation of provision .04(6)(f) 'proper cold holding' and/or 'proper hot holding' on next routine inspection may result in suspension of food service permit. Obtained copy of pH levels for pico and salsa for quality control.

  • Violation of Code: [.03(5)(c) ] Observed employee handle sausage and bacon, then touch ready to eat food. Person in charge discarded items. Advised PIC must change gloves and wash hands after touching potentially hazardous food and before touching ready to eat food.  Corrected On-Site.  New Violation.
  • Violation of Code: [.04(6)(f) ] Observed eggs, cheese, liquid eggs cold holding above 41F. PIC discarded items. Advised PIC when cold holding must PHF must maintain a temperature at or below 41F.  Corrected On-Site.  New Violation.
  • Remarks: Thermopin 32.0 Recommend to set up time as a public health control for eggs, liquid eggs and cheese. Recommend to randomly check the cook temperature of sunny side up eggs to make sure they are at or above cook temperature of 145. May want to add the consumer advisory for made to order eggs.
  • Have 90 days to have a new CFSM in place.

  • Violation of Code: [.05(7)(a)1 ] Interior of ice machine with mold like build up. Equipment food-contact surfaces and utensils shall be clean to sight and touch.  Corrected On-Site.  Repeat Violation.
  • Violation of Code: [.07(2)(l) ] Men and women restroom doors are not tight-fitting. A toilet room located on the premises shall be completely enclosed and provided with a tight-fitting and self-closing door.  New Violation.
  • Violation of Code: [.07(5)(a),(b) ] Damaged walls and floors throughout kitchen. Damaged & unsecured base cove throughout the kitchen. All physical facilities shall be maintained in good repair.  Repeat Violation.
  • Foods in walkin cooler were 41F - 43F; ambient air temperature measured 43F. Cooler temperature was turned down at the time of inspection
  • Unwashed produce stored above cooked chicken soup. Soup moved above produce at the time of inspection. Store unwashed produce below washed produce, cooked/ready-to-eat foods.
  • Disclosure statement "All steaks cooked to order" are not in all capital letters: in house menu & to-go menu. The disclosure and reminder statements shall be worded in legible type in all capital letters and no smaller than font size #8, or if displayed on a menu board shall be printed no smaller than the smallest lettering used for a menu item. Correct disclosure statement by next routine inspection cycle (7/1/2013).
