  • Violation of Code: [.03(5)(j)1&2 ] Observed employee drinks that were not a single service container with lid and straw. Person in charge discarded items. Advised PIC must have employee's place drinks in single service cup with a lid and straw.  Corrected On-Site.  New Violation.

  • Violation of Code: [.04(4)(m) ] Observed wet wiping clothes stored outside of sanitizer buckets. Person in charge put the wet ones in bucket to be washed and found new cloths. Advised PIC must store wet wiping cloths in sanitizer buckets.  Corrected On-Site.  New Violation.
  • Violation of Code: [.05(10)(e) ] Observed strainers stored on rinse aid containers and nest to mop sink below dirty mop. PIC moved containers. Advised PIC must keep clean containers away from contamination.  Corrected On-Site.  New Violation.
  • Violation of Code: [.05(6)(r) ] Observed single use items being reused to hold different type of food. PIC changed containers. Advised PIC single use items must only be used once.  Corrected On-Site.  New Violation

Observations and corrective actions:

  • Violation of Code: [.04(6)(h) ] Correctly date marked food items were not discarded on or before 7 day shelf life expiration date. Date marked food items need to be discarded after seven days. Food items found to be expired were discarded during inspection.  Corrected On-Site.  New Violation.
  • Remarks: FOOD IN THE FREEZER IS FROZEN SOLID. TIME CONTROL USED ON MOST ITEMS ON FRONT DISPLAY WRITTEN PROCEDURE AND LABELING IN PLACE.

Observations and corrective actions:

  • Observed employee handle cooked tempura vegetables with bare hands. Tempura was discarded and order was remade. Only handle ready-to-eat foods either with gloves or utensils.  Corrected On-Site.  New Violation.
  • Violation of Code: [.04(6)(d) ] Chicken soup in a large stock pot still above 41 degrees after cooling since 4/16/13. Discarded. Monitor all cooling to make certain that foods reach 70 degrees within 2 hours and 41 degrees within 6 hours.  Corrected On-Site.  New Violation.
  • Violation of Code: [.04(4)(k) ] Water that rice scoop was in was below 135 degrees. Water discarded and scoop placed with dirty dishes. Suggest keeping hot pot on a higher setting.  Corrected On-Site.  New Violation.
  • Remarks: NOTE: Beef and chicken both being cut on the same cutting board, but both cooked to above 165 degrees. Cut chicken, beef and seafood on separate cutting boards or wash, rinse and sanitize cutting boards between different types of food.
